Information received through this web site on the electronic pre-registration form will be kept confidential and used only for the purpose of creating a pre-registration for the patient. No other relationship or warranty is expressed or implied, and submitter agrees to hold Singing River Health System harmless for any harm caused by illegal interception of transmitted information.
